Vitamin D & Prediabetes

They symptoms associated with prediabetes and its affects in the development of long-term complications have been increasingly studies from a perspective of preventative medicine.  There has been strong evidence indicating Vitamin D’s role in the development of these complications as well.

“Vitamin D deficiency with DM type I, metabolic syndrome, obesity, cardiovascular diseases, hypertension and mortality associated possibly due to this vitamin role in insulin resistance, secretion and inflammatory process were reported.” (Parildar et al. 2013).

Studies also indicate that deficiencies of 25(OH)D decrease insulin secretion because calcium absorption is reduced.  It is believed that Vitamin D plays a role in either directly or indirectly affecting insulin resistance.
For this study, Parildar et al. (2013) serum 25(OH)D levels below 25 ng/ml were considered as a deficiency while above 25 ng/ml was considered to be sufficient.  Sufficiency levels were those levels that were able to keep PTH levels below 45pg/ml.  Those deficient in Vitamin D were supplemented with 2, 300,000 IU of D3 in one month followed by 800-1000 IU/day plus Calcium of 1200 mg/day as maintenance treatment. Individuals in the study were given medical nutrition therapy and a minimum of 150 minutes of aerobic exercises per week and measurements of the body were taken.

Results of this study showed with significance (p<.0001) that increased serum Vitamin D levels was associated with decreased fasting insulin and glucose.  Other studies aren’t as straightforward and indicate no change in insulin or glucose levels with increased Vitamin D levels.  Perhaps, the level of Vitamin D sufficiency is the issue here.  Sufficiency in this study was 25 ng/ml.  Others studies indicate that sufficient levels are at minimum 50 ng/ml for individuals to be able to build up Vitamin D stores and maintain those stores.
